Please discontinue my automatic payments to my credit card on file before the first of next month. I understand that my child's spot in the class/classes listed above will no longer be held after this month. Students withdrawing prior to the last class of the month are welcome to finish out that particular month. My child may still attend special events without being enrolled in a class and may return to the class program at any time where space is available. Registration fees are good through the entire session from the date of registration.

I understand that by submitting this form I am requesting for my dancer to be considered for a small group opportunity seen fit by the CKDW staff. I am not guaranteed placement into a small group simply by submitting this form.

Please know that small groups (4-9 dancers) are extra dances in addition to your group dances and are selected by the director or CKDW staff. Choreography can be set over the course of a weekend or spread out over a period of time. Choreographers, age range of dancers eligible per dance, and styles of dances are all selected by the CKDW DancElite/ADC faculty. Those who are interested in being considered for a small group must submit this interest form by the first week of class. Because these dances are EXTRA and not a guaranteed part of your company commitment, the cost for these extra dances will have to be paid upfront. Estimated costs for small groups are $450-­600 and include choreography fees, competition fees and costume fees. Accounts must be current in order for additional group dances to compete at competitions (please note that choreographers/teachers may not be present at competitions). If a dancer’s account is not up­ to ­date by the date of a scheduled competition, the dancer will not be allowed to compete their extra dances at that particular competition, and no refund of fees will be given. CKDW will not allow dancers to participate in extra dances if their account is not always current. Small groups will not perform in year-end showcases and/recitals.
